Medications tend to work better and cause fewer side effects in people with regulated sleep and stable blood sugar. Morning exercise raises dopamine and norepinephrine availability for the one to two hours that follow, a window that overlaps usefully with when focused work needs to happen. A protein-forward breakfast stabilizes glucose through the morning. Consistent sleep timing, which is frequently disrupted in ADHD, reduces the attention and impulse control deficits that poor sleep reliably causes, separate from the ADHD itself.

Bipolar Spectrum

Several supplements that are safe for unipolar depression carry meaningful risk in bipolar disorder. St. John’s wort, SAMe, and stimulating herbal preparations can trigger hypomania or disrupt the sleep that mood stability depends on. This happens regularly enough that any new supplement should be reviewed with the prescribing clinician before starting, not mentioned at the next appointment.

Well supported: Exercise for mild to moderate depression and anxiety (effect size comparable to medication in multiple RCTs). Mindfulness-based cognitive therapy for relapse prevention in recurrent depression. Bright light therapy for seasonal depression. EPA-dominant omega-3s as an adjunct for depression. CBT-I for insomnia.

Promising but variable: Yoga and tai chi for anxiety, PTSD, and sleep. Acupuncture for anxiety and insomnia in selected patients. Probiotics for mood and anxiety, particularly with coexisting GI symptoms. Nutrient repletion (vitamin D, B12, folate, iron) when there is a documented deficiency.

Individual response varies considerably, which is why clinical monitoring matters more than following a protocol. What works well for one person, even someone with an identical diagnosis, may not work at all for another.


Safety

Three rules prevent most problems:

Discuss every supplement and herbal product with the prescribing clinician before starting. The interactions are real, not theoretical, and are not always intuitive. St. John’s wort interacts with dozens of common medications.

Add one change at a time. Three new supplements started simultaneously make attribution impossible when something helps or causes a problem.

Sequence matters. Sleep timing, morning light, and a brief daily nervous system practice tend to stabilize the biological environment that other interventions require to work. Building exercise, therapy modalities, and targeted supplements on that foundation produces better results than trying to address everything at once.
